**Responsibilities**
The Transplant Coordinator is a member of the team responsible for coordination of the clinical process from patient referral, evaluation, acceptance, treatment and follow-up. Incumbent serves as a resource to staff and multi-disciplinary team and community caring for program patients.
+ Demonstrates knowledge and skills in the care of patients from adolescent to adult, and will play a role in the holistic evaluation of patient(s).
+ Skills in the area of critical thinking, interpersonal communication, conflict management, problem solving, autonomy and decision-making are highly desirable.
+ Maintains program compliance and QAPI. Complies with regulatory, governing and other external agencies requirements in the area of quality, data reporting, accreditation surveys, and inspections.
+ Complies with all state and federal regulations.
+ Complies with all policies, procedures and standards to ensure consistent application and effective operations.
+ In collaboration with the program Medical Director and administrator, provides for timely data input and management.
+ Assists in developing policies and procedures, order sets and pathways to improve overall quality of care and program development.
+ After hours and weekends required to meet patient and program needs.
+ On call duties as assigned

**Qualifications**
**MINIMUM**
Nursing degree.
Three years experience with patients in a related setting and/or three years experience in an intensive care, acute care, telemetry or medical/ surgical setting.
Working knowledge of regulatory compliance and survey processes.
Joint Commission and CMS standards.
Working knowledge of QAPI process.
Strong verbal, written communication skills.
Proficient computer skills.
Performance improvement tools and methodologies such as PDCA, Six Sigma, Lean, etc.
Current Arizona RN license (or compact state).
**PREFERRED**
BSN
Critical care/cardiothoracic experience.
Program Management/Development.
**Pay Range**
$42.69 - $61.91 /hour
