**Responsibilities**

This position reports to the Senior Director of Intensive Care Services. This position is responsible for planning, developing, implementing, coordinating and evaluating palliative care services in collaboration with the Sacramento area Mercy Hospitals and Dignity Health, in a manner consistent with the Mission and Philosophy of Dignity Health. Included in this responsibility is developing and maintaining strong relationships with physicians and hospital staff to deliver palliative, comfort care to seriously ill and dying patients. In addition, this position is responsible to collaborate with related healthcare practitioners, the patients and their significant others in assisting with admissions of appropriate patients to hospice service. This position is responsible for the enhancement and promotion of palliative care in the hospital, assuring educational opportunities; standards and program development; quality improvement activities; consultation and care management. This position is responsible for promoting high quality, cost effective care for inpatient hospice patients and other palliative care patients served in a highly regulated and fiscally challenging managed care environment.

**Qualifications**

**Required**

+ Minimum of five (5) years RN experience, preferably in medical-surgical services, oncology nursing, hospice, geriatrics, intensive care, and/or geriatric-psychiatry

+ Current CA RN licensure and AHA BLS

+ BSN degree or additional 5 years' related experience in lieu of degree.

**Preferred**

+ Specialty in medical/surgical nursing, palliative care, oncology, hospice or home care

+ Certified Hospice and Palliative Nurse (CHPN) certification

**Skill Specifications**

+ Demonstrated clinical standards for hospice and palliative care practice, community resources related to palliative care or hospice programs.

+ Demonstrated knowledge of Medicare and MediCal regulations, Title 22, NHPCO and the Joint Commission standards, for Hospital, Hospice and SNF, RCFE regulations.

+ Ability to demonstrate competency with death and dying process, terminal and chronic illness, grief, and coping skills.

+ Effective Interpersonal Skills, problemsolving techniques including conflict management skills.

+ Excellent oral andwritten communication skills

+ Expert pain and symptom management, administration and therapeutic effects of medications. Concepts of managed care. CQI and QA methodologies.

+ Effective computer skills.

**Pay Range**

$54.12 - $78.47 /hour
